Premature Ejaculation: Causes and Treatment

Premature ejaculation refers to a condition where ejaculation occurs too quickly during sexual activity, failing to satisfy the partner. Just as our heartbeat is controlled by the autonomic nervous system and not by conscious effort, the nerves in the penile corpus cavernosum that are related to erection are also governed by the autonomic nervous system.

Major causes of premature ejaculation include psychological trauma, dysfunction of the central and peripheral nervous systems, excessive alcohol consumption, smoking, and various other factors.

The main treatments for premature ejaculation are as follows:

  • Dorsal nerve block: The most effective premature ejaculation surgery method that controls the sensitivity of the glans by blocking the dorsal nerve.

Premature Ejaculation Treatment: Dorsal nerve block surgery

Many patients with premature ejaculation are found to have heightened sensitivity in the glans compared to others. Dorsal nerve block surgery is the most effective procedure for delaying ejaculation by reducing the sensitivity of the glans through blocking the dorsal nerve, which is responsible for sensory input.

For example, applying an anesthetic ointment to the glans before sexual activity can delay ejaculation. Based on this principle, dorsal nerve block surgery involves partially severing the nerves responsible for glans sensitivity to reduce overstimulation. The procedure is effective in approximately 90% of patients and is performed under local anesthesia, taking about 20 to 30 minutes. After a recovery period of around two weeks, patients can typically resume normal sexual activity.

Premature ejaculation is most prevalent in young men in their 20s and 30s, with the main cause being heightened sensitivity of the glans and penis . After a dorsal nerve block is performed to dull the sensation, the area is sutured with dissolvable threads which dissolve in about 1 to 2 weeks, and sexual intercourse is possible 2 to 4 weeks after the surgery.

Who requires Premature ejaculation surgery?

Premature ejaculation surgery does not dull all sensation in the entire genital area, instead selectively dulling sensation in select areas that are most stimulated by friction during sexual intercourse, and are therefore most likely to lead to premature ejaculation.

The surgical site generally has reduced sensation, so there is minimal pain. Although there may be a slight stinging sensation in the glans during the early stages after surgery, this gradually subsides over time.

We recommend premature ejaculation surgery to the following people:

  • If you ejaculate in less than 5 minutes during sexual intercourse
  • If your partner is not satisfied during sex
  • If you feel severe stress and psychological pressure due to repeated premature ejaculation

Although there are some differences between patients, it is effective in about 90% of cases.

The stitches will dissolve and disappear in about 1 to 2 weeks after the surgery, and sexual intercourse is possible after 2 to 4 weeks.
